Permission to Skate Forms (PTS)

A “Permission to Skate” is required should a player registered with the Oakville Hornets wish to attend rep tryouts or practice with another Association. A “Permission to Skate” is time-limited and valid only for the specified period of time recorded on the form. A separate form is required for each Association the player wishes to try out for. All requests for a “Permission to Skate” form must be sent to Elsie (info@oakvillehornets.org), stating the following:

  1. Subject line - Permission to Skate Request
  2. Girl's full name
  3. Team and Association Name for which she is wishing to try out for (e.g., Brampton Canadettes Atom AA)
  4. Time Frame (i.e., start and end date “Permission to Skate” is required – note the starting date cannot be earlier than April 14, 2010).

Requests must be made in writing by Friday, April 9 . “Permission to Skate” forms will be available for pickup starting on Tuesday, April 13 from 6:00 pm in the Hornets office.

As per OWHA rules, "No 'Permission to Skate' forms may be used for the period between January 1st and two days after all the Provincial Championships for any teams during the current season".

Release Forms

It is OWHA policy that female players remain automatically tied to their OWHA association, until a formal release from that organization has been received. This is the case for both house league and rep players, even if your registration has lapsed.

In the event that any Oakville Hornet Player plans to move to a new association within the OWHA (e.g. because of a family move, selection to a rep team in another organization, etc.), a "Player Release" form is required from the Oakville Hornets prior to registering with the new organization. "Player Releases" must be approved by the Executive.

All requests for "Release" forms must be made to the office via e-mail (info@oakvillehornets.org) stating the following:

a) Subject Line - Release Form Request
b) Girl's full name
c) Contact information for pickup, if other than email

It is your responsibility to request a "Permission to Skate" or a "Release" on a timely basis. Please remember that we are a volunteer organization and that the office is staffed on a part-time basis.

While a PTS is not required to attend try outs of a boys teams, some associations require these be presented. It is recommended that anyone attempting to try out for a boys team confirm beforehand the procedures and requirements of that organization.
