All coaches, assistant coaches, trainers and other staff:
All coaches, assistant coaches, trainers and other staff who are on their team's
OWHA roster need to have a police check completed and the original submitted to the
Oakville Hornets office as soon as possible (no photocopies will be accepted). Click here for an outline of security requirements.
The criminal reference
letter (click here to download the .doc Word file) is to be submitted when applying for the police check. Vulnerable Sector Screening must be requested and is required on the
police check for the form to be considered complete.
